This is a full-time, 40 hour/week role with working hours from M-F 9am-5:30pm ET. Weekend Availability: This position will require weekend work based on business needs, operational priorities, or member demand. Team members will be provided advance notice whenever possible, and schedules will be communicated in accordance with organizational guidelines. A bit about this role: The Clinical Associate is an integral part of Devoted Medical’s interdisciplinary virtual and home-based care team. Our team delivers compassionate, comprehensive care to Devoted Health members through telehealth, Care on Demand, and longitudinal programs that support our most complex and frail members. This role represents the future of healthcare delivery — bridging technology, empathy, and precision to make every interaction seamless and human. You’ll serve as a trusted partner to both our members and our clinicians, ensuring that care is coordinated, personalized, and easy to access. Our Clinical Associates are: The architects of trust. What you’ll do Responsibilities may vary based on the clinical program supported (Digital Care, Care on Demand, Primary Care, Speciality Care). Examples include: Pre-Visit Preparation Guide members through onboarding for virtual care, helping them feel comfortable and confident using digital platforms Ensure the provider has all necessary information for an efficient, personalized visit Post-Visit Coordination Manage orders, referrals, and follow-ups to close care gaps and ensure continuity Coordinate with home health partners, DME suppliers, pharmacies, and labs Notify members of results, reinforce care plans, and ensure provider follow-up Digital Care Navigation Support real-time telehealth workflows, helping triage member needs between clinicians, nurses, and care guides Monitor and resolve member inquiries from multiple digital channels (chat, phone, portal) Identify and escalate urgent clinical needs appropriately Utilize AI agents and provide feedback to prompts Patient Advocacy & Education Serve as the member’s advocate, ensuring they feel heard, cared for, and supported Reinforce health education materials and program resources Follow up on transitions of care, ER discharges, and other moments when members need us most Attributes for success You will thrive in this role if you are: Passionate about helping others improve their health and navigate care with compassion and empathy Experienced in care coordination, especially for older adults or those with complex needs Digitally fluent and quick to learn new tools and systems (EHR, chat, telehealth, and scheduling platforms) A strong communicator, able to build trust through video, phone, and written communication Calm under pressure, skilled at easing anxiety and uncertainty for members and colleagues alike Entrepreneurial, self-directed, and comfortable working through ambiguity Highly organized, able to manage multiple priorities and maintain precise follow-up Required skills and experience MA Certification (highly preferred; expired certifications accepted). If not currently certified, equivalent MA training or experience is required (please provide transcripts or proof of completion) 2+ years of patient-facing experience, with a strong preference for telehealth, primary care, or urgent care/hospital backgrounds This is a full-time, 40 hour/week role with working hours from M-F 9:00am-5:30pm ET.
