Assessment Coordinator Opportunity - Weekend Day Shift Cumberland Hall Hospital is a 97 bed, licensed, acute psychiatric hospital located in beautiful Hopkinsville, KY. Cumberland Hall features individual units for youth, adults and active-duty military, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. On average, over 1500 patients receive care from our compassionate health care team each year at Cumberland Hall Hospital. Website: https://cumberlandhallhospital.com/ Join our team as an Assessment Coordinator and play a critical role in guiding patients through the admissions process while ensuring clinical appropriateness, continuity of care, and effective coordination across the treatment continuum. This position offers the opportunity to use strong clinical judgment, communication, and leadership skills to support patients, families, and interdisciplinary teams in a fast‑paced, mission‑driven healthcare environment. Job Duties to include: Conduct comprehensive clinical or psychosocial assessments to determine appropriateness for admission and ongoing level of care in accordance with regulatory and organizational standards. Coordinate the admission process by obtaining required documentation, collaborating with physicians and interdisciplinary team members, and ensuring timely and accurate entry of patient information into clinical systems. Serve as a primary liaison for patients, families, and referral sources, providing clear communication, education, and support throughout the assessment and admission process. Manage and prioritize incoming referrals and calls, assess urgency, and facilitate appropriate placement or next steps across the continuum of care. Education
Associate Degree in Nursing (ADN) or Master of Social Work (MSW) required.
Experience
A minimum of one (1) to three (3) years of relevant professional experience as either a Registered Nurse or Social Worker is required. Licensure: Active RN licensure is required for nursing candidates. Social work licensure is preferred but not required for candidates with a Master of Social Work (MSW) degree. Shift Available: Friday, Saturday and Sunday; 6:30 AM - 7:00 PM.
