This role cultivates the organization's social community through community advocacy programs and active engagement with Humana Healthy Horizons in Kentucky enrollees as well as members of the general community. The Community Engagement and Field Care Worker builds visibility and credibility of organization's products and services throughout community via a variety of interactions. Strategizes methods to scale communities and drive greater connections among community members. Engages new community members. Understands own work area professional concepts/standards, regulations, strategies and operating standards. Makes decisions regarding own work approach/priorities and follows direction. Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation Use your skills to make an impact Required Qualifications Must reside in the Northern Kentucky region. Two (2) or more years of professional human services experience OR recent service in the same role, working in-person with consumers conducting assessments and/or developing care plans. Minimum of one year of experience in community outreach and/or marketing and sales Community Health Worker Certification must be obtained within 6 months of hire date. Excellent written and verbal communication skills Proficient in Microsoft Office (Excel, Word, PPT). Experience using a variety of electronic information applications/software programs. This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits. To ensure Home or Hybrid Home/Office associates' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office associates must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested. Satellite, cellular and microwave connection can be used only if approved by leadership. Humana will provide Home or Hybrid Home/Office associates with telephone equipment appropriate to meet the business requirements for their position/job. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.
Preferred Qualifications
Bachelor's Degree in Social Work or Health related services Bi-lingual: Spanish Home Health experience Knowledge of community health and social service agencies and additional community resources.

Workstyle: Field. Field workstyle is defined as associates performing their core duties at one or more non-Humana locations. Work Location: must reside within 50-mile radius of Northern KY. Travel: 60-80% within 50 mile radius of Northern Kentucky. Typical Work Days/Hours: Must be available to work Monday - Friday between the hours of 8am-6pm Eastern Time Zone (EST) but be flexible with your hours based on business needs.
