The Role
We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey.
In this role, you will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care. This role requires a high level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams to deliver timely, patient-centered, and scalable remote care.
Although this position is fully remote, it is a highly collaborative, fast-paced clinical role with clear expectations for productivity, responsiveness, and quality.
Licensure Requirements
Required: Active Compact Multi-State RN license
Required: Active NP licensure in CA, WA or AZ
Licenses must be active and in good standing at time of hire.
Schedule Requirements
This is a Monday through Friday role. Preference will be given to candidates who are available for a later weekday schedule, including coverage until 11:00 PM Pacific Time.
What You'll Do
• Manage a panel of patients receiving remote care, making timely clinical decisions based on evidence-based guidelines for heart failure, hypertension, and type 2 diabetes.
• Deliver high-quality, evidence-based care by following established clinical protocols and workflows while applying sound clinical judgment to individual patient needs.
• Initiate and titrate guideline-directed medical therapy (GDMT) while collaborating closely with physicians, RNs, and care teams to optimize outcomes.
• Respond promptly to clinical escalations generated through remote patient monitoring, including abnormal vitals, symptoms, and laboratory results.
• Provide both synchronous and asynchronous patient care, including reviewing patient-generated health data, responding to remote patient monitoring alerts, communicating through secure messaging, and conducting scheduled virtual visits when appropriate.
• Deliver safe, efficient, high-quality care while consistently meeting established productivity and documentation expectations.
• Adapt quickly to evolving workflows, software enhancements, and operational processes as we continuously improve our care model.
• Use multiple technology platforms confidently and provide feedback that helps improve our clinical operations and technology.
• Collaborate across clinical and operational teams to identify opportunities to improve patient care, efficiency, and scalability.
