The RN's primary role is to provide personalized patient care; support the provider's practice by performing clinical duties and assisting in the coordination of patient care.  The RN's communication, conduct and performance should support the clinics efforts.

Documents patient information in the electronic health record (EPIC) following standard work guidelines Assesses and triages patient care needs.  Suggests solutions to patient care problems & complaints Identifies & recommends solutions to departmental problems Assists in assessing departmental function and productivity & recommends changes as necessary Assesses patient/family learning needs and coordinates and carries out teaching plan Coordinates care for Heart Failure and general cardiology patients including telephone triage, advising and consultation with patients, nurse managed care coordination, patient/family education, and in office cardiovascular procedures Coordinates care across the continuum for specialty services internally and externally to manage heart failure complex care Manages care coordination with cardiomems patients and
